Please use bellow form to report any harassment, atrocities, violations of human rights, detainments, or deportations of Afghans in Afghanistan, Pakistan, or elsewhere. Please include as much detail as possible so that appropriate parties may investigate and take action.

#AfghanEvac may flag your personal case information for follow-up with the State Department. If you do not wish to have your personal information shared with the US government, do not provide case information below.

#AfghanEvac will retain this information only as long as necessary to create trend reports or for 6 months from date of receipt, whichever occurs earlier, and then securely delete the information. The State Department will retain the information in accordance with their policies.

The purpose of this form is to capture and share with relevant U.S. and international authorities aggregate information related to atrocities and other incidents that may be impacting Afghans.
